Practices within the expressed limits of established policies and procedures, securing approval from the Head of Pharmacy for any deviations from the policies and procedures.- Ensures the provision of products with optimum quality by following Standard Operating Procedures (SOPs) and quality assurance testing _(if required)._- Works closely with pharmacists or supervisors in other units and Head of Pharmacy in day-to-day issues. Reports to the Head of Pharmacy regularly.- Reviews prescription orders and ensures that the medications prescribed are safe and appropriate for the intended use. These include potential drug side effects, harmful interactions with other drugs and the suitability of treatments for patient with particular health conditions.- Verification of EMR to ensure that medications prescribed are safe and appropriate for the intended use.- Uses his or her knowledge to advise patients about the safe and appropriate use of medicines they have been prescribed. Provides counselling on medications or medical devices used to the patient e.g. method of administration or use, effects, dosage regimen, particularly those who require complex drug therapy, as well as provides bedside dispensing and counselling.- Works collaboratively with other healthcare professionals to devise the most appropriate drug treatments for patients.- Works closesly with physicians, nurses and other healthcare professionals to ensure the delivery of safe and effective drug treatment.- Contacts physicians to clarify any discrepancies or issues that appear in the prescription.- Responds to any medication-related queries within the hospital, other hospitals and the general public.- Supervises the execution of duties by staff within the unit that you are in-charge of e.g. Outpatient pharmacy, Inpatient pharmacy or Cytotoxic Drug Reconstitution (CDR).- Ensures that high professional standards are maintained.- Manages the work of the team or unit members to ensure optimum use of manpower.- Formulates and supervises the compounding of extemporaneous medicines and lotions, as well as pre-packing of medicines.- To review and amend extemporaneous drug formulary and lotion formulary as required regularly.- Participates in the training or teaching of pharmacy staff, trainees and other healthcare professionals as required by the management (internal and external pharmacy-related CPD programmes).- Maintains records on transactions of all controlled drugs (i.e. Psychotropic Drugs and Dangerous Drug) as required by the Malaysian laws and regulations.- Ensures that medicinal products are stored and transported appropriately accordingly to the manufacturer's recommendations to ensure freshness and potency.- Ensures that the medications reach the patient in the correct form and dose.- Monitors consumption of stocks in the pharmacy, wards and units and takes appropriate measures to ensure cost-efficient use of stocks and minimise wastage.- Strictly maintains the patients confidentiality.- Promotes and maintains quality in all activities and ensures continuous quality improvement measures are followed.- Complies with and ensures that those directly supervised comply with department and hospital policies and procedures.- Participates in Pharmacy department meetings, Pharmacy and Therapeutic Clinical meetings, other departments or hospital committee meetings and quality improvement activities.- Participates in the writing, review and amendments of department Policy and Standard Operating Procedures (SOPs).- Maintains statistics and appropriate departmental documents and submits reports as requested by the Head of Pharmacy.- Assists the Head of Pharmacy in staff appraisal for staff under direct supervision _(in consultation with the supervisor or Head of Pharmacy, if required)._- Undertakes other relevant tasks as may be directed by the Head of Pharmacy from time to time.**Job Requirement**:- Possess a Bachelor's degree in Pharmacy (or above).- Registered with Malaysian Board of Pharmacy.- Possess Poison License A and Annual Retention Certificate.- Possess 1-3 years of working experience.- Able to be on-call _(after office hour)._- Possess good communication and interpersonal skills in both English and Malay _(proficient in Chinese or Tamil is an added advantage)_.- Able to work independently and accountable.**Benefits**:- Free parking- Health insuranceSchedule:- Rotational shift
