Identifies and implements interventions to improve Quality/Stars and HEDIS measures. Responsible for overall data tracking, reporting of program results, and providing continued feedback to management and leadership. Responsible for building direct relationships with Primary Care Providers and their clinical team to ensure successful outcomes for effective patient centered care plans. Knowledge of and ability to collaborate with Freeman and community resources. Participate, attend and represent at various meetings and community events.RequirementsCurrent Missouri Registered Nurse license or current Registered Nurse license from a compact state.
