**DUTIES & RESPONSIBILITIES****1.0 Direct Comprehensive Care**providing holistic care for patients and their families, visitors.1.2 To conduct a thorough nursing assessment of patient's physical, psychosocial,cultural and spiritual needs.1.3 To formulate, implement and document a holistic plan of nursing care that iscommunicated and clarified with the patient / family.1.4 To prioritize and organize diagnostic tests and procedures.1.5 To demonstrate "hands on" clinical skills specific to his/her area of practice.1.6 To ensure all patients are treated as individuals, irrespective of color or creed.Displays sensitivity to individuals' religious and cultural practices.1.7 To evaluate patient responses to interventions in order to determine progresstoward achieving expected outcomes and modify the nursing plan of care accordingly1.8 To provide a comprehensive report when transferring accountability of a patient'scare to another nurse and / or facility.1.9 To document patient history, physical examination data, nursing care plan andinterventions and the patient's responses in the medical record in a timely mannerin accordance with nursing Standard Operating Procedures (SOP).1.10 To be accountable for own professional actions**2.0 Supportive Care**2.1 To establish a caring and therapeutic relationship with patients demonstrating trust, respect and honesty2.2 To demonstrate excellent communication skills during all phases of the therapeutic relationship2.3 To create a positive environment by listening and offering support and maintaining confidentiality at all times2.4 To assist patients to identify psychosocial needs, concerns and offers appropriate resources through information provision, education and referrals**Navigating the System of Care**2.1To understand the impact of the disease experience and plan how to optimally guide the patient and family throughout the medical treatment2.2 To develop a collaborative partnership with the patient and family that is respectful of their needs, wishes, knowledge, experience, values and beliefs2.3 To be knowledgeable about the resources provided by any disease program in the hospital2.4 To organize the interdisciplinary plan of care with other services and team members as required to facilitate the seamless movement of the patient through the healthcare system2.5 To facilitate the process of patient education which consists of assessment of learning needs, the development of learning objectives with patient, implementation of teaching and evaluation of learning2.6 To provide health counseling, navigation through treatment and interpretations of written as well as media / web based patient resources2.7 To plan, deliver and evaluate education directly or on a consultative basis for individual and or groups of patients with the same disease and their families' in partnership with the interdisciplinary team**Professional Practice**2.1To participate in professional and specialty nursing organizations2.2 To support development of and participates in clinical relevant project work2.3 To advocate for and support the development, implementation and evaluation ofquality improvement initiatives (Policy & procedures changes, standards, protocols, clinical pathways and medical directives).2.4 To engage in self-appraisal by being proactive and innovative in meeting own learning needs based on patient population and changes in practice.2.5 To perform the reflective practice, which includes the required capacities of self- awareness, self-knowledge, empathy, awareness of boundaries and maximum scope of the professional role2.6 To engage in reflective practice by maintaining knowledge and skills that will enhance / advance her / his professional nursing practice2.7 To be a role model for excellent nursing practice through demonstration of expertise2.8 To act as a resource to individuals and groups within the hospital and community agencies to improve care and nursing practice2.9 To act as a preceptor or mentor to staff / students, providing them written or verbal feedback as necessary2.10 To recognize ethical dilemmas and works through them using resources and logical approach2.11 To demonstrate ethical behavior (ie. honesty, integrity)2.12 To recognize when own values and beliefs conflict with the patient's and be able to demonstrate conflict resolution skills**Evidence - Based Practice (EBP)**2.1 To demonstrate knowledge relevant to her / his professional practice by providing theoretical or scientific physiological and or psychosocial rationale2.3 To participate in nursing research initiatives.**Job Types**: Full-time, Permanent**Salary**: RM1,800.00 - RM6,000.00 per monthSupplemental pay types:- Overtime payAbility to commute/relocate:- Shah Alam: Reliably commute or planning to relocate before starting work (required)**Experience**:- relevant: 3 years (prefe
